Advancements in the Treatment of the Aging Face with Volume Restoration: When patients come to see Dr. Roberto Garcia for consultation, they usually come with a good idea about the concerns they have and areas of the face they would like addressed. We take this opportunity to delve deeper with the patient and explain in detail what has occurred structurally and anatomically to cause the aesthetic changes they are seeing in the mirror. When the face is young, fat is distributed evenly throughout.As the face ages, fat dissipates, moves, and gets redistributed.Muscles under the skin sag, get baggy and result in the jowly appearance around the chin and jawline. Facelifts and other lifting procedures only address the descent and "drop" of facial tissues and muscles. Generally speaking, these procedures fail to address the patient's changing facial shape due to loss of volume. Present trends and those procedures offered by Dr. Garcia will often address the descent of tissues issue AND the loss of volume.Volume loss will usually involve the use of the patient's own fat. At times, dermal fillers may be used instead of fat. When the aging face is addressed using this 2 pronged approached; a lifting procedure and volume restoration, the goal of natural, facial rejuvenation is much more achievable. The youthful face is “heart shaped” with 2 imaginary arcs that give the face a youthful appearance. The temple arc gives the face the lateral projection and the anterior arc provides the forward projection. Both of these projections help make the face appear youthful by distributing volume in the upper 2/3 and narrowing towards the lower 1/3 of the face. The volume augmentation procedure (Contoura procedure) was specifically designed to re-create the youthful “heart shape” to the face. By utilizing one's own fat to re-establish the volume to the upper 2/3 of the face, the changes can be dramatic. The face once had fat in this region of the face, and as such, the rate of success of fat transfer to this area is very high. The process of adding volume is straight forward. Patients can actually drive themselves to the office, have the procedure done, and go home right after. Patients may resume their everyday activities the next day. During the many years performing this type procedure, Dr. Garcia's patients experience little to no discomfort.
